When patients have gained maximal mobility and the ability to return home with some independence, the social worker and discharge planner will plan discharge. A multi-disciplinary Care Conference is held with family members to ensure that everyone is in agreement of the course of action recommended. The discharge planner will schedule any needed services, such as home health care services and outpatient therapy, if necessary.
